Filing Requirements

To file credentials to perform marriages in Minnesota you will need to:

1. Complete the Certificate of Filing form. Signature on form must be notarized
2. Provide the original credentials of license or ordination
3. Provide check or money order for filing fee of $20
4. Officiant must be 21 years of age or older
5. Mail completed form, credentials and fee to:
   Polk County Recorder
   P.O. Box 397
   Crookston, MN 56716
6. We will return your credentials and form to you after filing has been completed.
