Polk County Public Health is a local health department committed to fostering the health and well-being of all Polk County residents. Our offices are located across Polk County in Crookston, East Grand Forks, and McIntosh and where you’ll find approximately thirty staff out in our community working with local individuals and community partners who share a goal of promoting and improving health for all.

Polk-Norman-Mahnomen Community Health Board
Under Minn. Stat. § 145A, a Community Health Board is the legally recognized governing body for local public health services. The Polk-Norman-Mahnomen Community Health Board (PNM CHB), a multi-county Community Health Services entity, brings together the expertise and resources of Polk County Public Health and Norman-Mahnomen Public Health, serving the counties of Polk, Norman and Mahnomen in Northwestern Minnesota.
