Polk County Public Health provides high-quality, confidential sexual and reproductive health care to those living in Minnesota at low or no cost. Services are available at our Crookston, East Grand Forks, and McIntosh offices, as well as on the UMC campus. No one is turned away due to inability to pay.

Polk County Public Health provides a full range of sexual and reproductive health services, including:

Fees & Payment
Please note: Nobody will be turned away for inability to pay.
Minnesota Family Planning Program (MFPP)
The Minnesota Family Planning Program (MFPP) provides confidential coverage for family planning services, related supplies, and transportation to and from family planning appointments.
MFPP may be an option if you:
- Live in Minnesota, and
- Are uninsured or want confidential insurance for family planning services, and
- Are not enrolled in MN Medical Assistance, which already covers these services
Coverage Options
- Short-term coverage available at certain clinics
- Up to one year of coverage with a completed application

- Proof of income for the past 30 days

We offer testing for a wide range of sexually transmitted infections, including:
- Bacterial Vaginosis (BV)
- Chlamydia
- Genital Herpes
- Genital Warts/HPV
- Gonorrhea
- Human Immunodeficiency Virus (HIV)
- Human Papillomavirus (HPV)
- Molluscum Contagiosum Virus (MCV)
- Pubic Lice
- Syphilis
- Trichomoniasis
GYT (Get Yourself Tested) Kits
GYT (Get Yourself Tested) kits are urine-based STI test kits that screen for Chlamydia and Gonorrhea. These kits are designed to be simple, private, and convenient.
How it works:
- Pick up a kit at one of our locations
- Collect your urine sample at home or in our public restrooms
- Return the completed kit in the designated collection bin

When to Get Tested
Many STIs do not cause symptoms, which is why testing is so important.
- Testing is recommended every 6–12 months if you are sexually active
- Testing is recommended with every new sexual partner
- You may need testing sooner if you believe you were exposed
STIs can be spread through sexual contact or the exchange of bodily fluids. Condoms are very effective at preventing many STIs when used correctly every time.
If you think you may have been exposed to an STI, talk with your health care provider about testing options.
